General Information
4. Absolutely NO makeup on eyes or face
5. If you arrive with makeup on that prevents me from providing a safe and clean service you
will be charged $18 for a lash bath. *please do not try and rub the makeup off yourself, that
doesn’t work! They need to be cleaned thoroughly*
6. Hands must be washed and sanitized before your service begins
7. 40% OF LASHES MUST BE PRESENT FOR A REFILL
8. ARRIVE ON TIME: 10 minute GRACE PERIOD. Your appointment will be canceled after 10
minutes (especially if you do not call to notify the technician about your lateness). Based on the
technician’s ability/discretion a $20 rush fee will be added to your total if the service can still be performed.
9. My squeeze in fee is $30 extra. That also includes before opening and after closing.
10. If you experience a medical issue due to the service, the client will receive a 50% refund.

Cancellation Policy

12. A $35 late cancellation fee is non refundable, Eyes Ink does not take deposits therefore; this
cancellation fee is required and will be charged on your card via square. NO EXCEPTIONS
-After 2 missed services, you will no longer be able to book online and will need to schedule
with a technician over the phone or at the studio.

Additional Information
13. If you are a new client please note that there is an intake form that must be filled out. Please
give yourself enough time to fill everything out in order for us to start on time.
14. If you need to bring additional guests please let the technician know in advance.
15. Must be 18+ years old to book (adult consent is needed for ages under 18)
16. If you need to notify me contact 215-337-5937. Eyes Ink NO LONGER has a text line
before/after hours. Responses to all messages will be within 24-48 hours.
